Patients are seen by appointment. Our automated calling system calls to confirm most appointments two days in advance. Cancellation should be made 24 hours prior to the appointment. A fee may be charged for missed appointments.
Social Security Numbers:
We handle patient’s social security numbers and personal information in a confidential manner. This is required information of all our patients and is used for insurance and billing purposes. In addition, it is required by a lab if a biopsy is performed and for several other procedures.
Currently we participate with several insurance companies including Blue Cross Blue Shield, Medicare w/ Secondary Insurance, Medicare Advantage Programs and Tricare. Remember, insurance is a contract between you and your insurance company and is the patients responsibility to make sure that our providers participate with your insurance company. All insurance deductibles and co-pays will be collected at the time of service. If we are non-participating with your insurance plan, then you will be responsible for payment in full at the time of service. In addition, all cosmetic and non-covered procedures must be paid in full at the time of service. Our office will provide you with the necessary claim form to submit to your insurance company. All children under the age of 18 must be accompanied by a parent or guardian. The child’s social security number will be needed at the time of the appointment.
